There are still 2 corrections needed on the CHED form 9.

‘1. Under credit/group the #2 is running into #4. I think there is still room to move #3 & #4 & #5 to the next column(s) over.
‘2. The Certificate of Registrar statement is supposed to only be indented on the first line. The second line should be flush to the left.

We have tried to work with you on this to make it what you want, in order to simplify your processes. But, since this is a custom report just for your school, we must prioritize other needs, and we don’t have time to constantly be changing it. Please come up with your final list of changes that you can live with, and we will try to work them in when there is time.

Dear Kindra,
I’m sorry that we have given you the impression of constantly changing our form requests. Actually, we have been asking for the same changes since the beginning. I realize that you have felt that you already made the changes we have requested, but please understand that the forms we are dealing with here are for the government and therefore must be exact. Any deviations from the required format require us to fix them item-by-item before we can submit the forms to the government, which has very specific expectations. Though I understand it takes a lot of your time to get the forms into the required format, please recognize that any deviation takes hours upon hours of our time as we have to “tweak” the forms for every student every year.
As of now, there are only two remaining changes that we are asking for. These were things we asked for from the beginning.
Actually, last May 2018 I did come up with my final list of alterations to the CHED 9 form.
Judy sent it to you on July 4, 2018.
When we did not hear back from you we sent another request on August 9, 2018.
Per your reply on August 13, 2018 that you were very busy, we gave you time, since we did not expect to need the form until now, February 2019.
Since the changes had still not been made, Judy sent the original request again on January 16, 2019.
You then did make some of the changes.
The request from February 11, 2019 are not new items. They are actually some of the same original items I’ve been trying to get corrected since the beginning. But they have not been corrected the way I had asked them to be. Each time I have provided attachments that show what I am specifically asking for.
I have attached two files again. One is the current view and one is the correct view of what it should look like. Here are the two items that still need to be corrected:
