I found a bug in the application form process. 

The field "district" was added in the last update, when this field is selected and part of the application form, when you evaluate a person and in the church section, the following error occurs. See image.

I did it in two ways, when you remove the field as a requirement then the evaluation completes. When the field is on it gives the same error, either when you still need to lookup the church or even when the church is already founded and added and the evaluation completed as well. The error is consistent.
