There was a change to the system to remove the ability to directly create a user. Users and their access is created in a couple key ways. Though I am assuming you want to create a user for a staff member, below I am putting the ways users are created and their access.
Online Application: A new online application creates a very limited login for the applicant to work on their application and see the submission process.
Prospect: Once a application is cleared by Admissions the Applicant login is extended so that they can access the admissions portal.
Student: When a Prospect becomes a Student they then have access to the Student Portal and the functions that are enabled at your institution.
HR Employee: HR can make a person an employee. This creates a user that can be granted many different types of Staff portal access depending on your institutions Permission Groups in System Administration. Making an employee active is what creates the user if they are not already a user from Student. Inactivating an employee will also remove that employees access to the Staff portal and the Permissions Groups that were assigned to their user account.
Instructor: Instructor would first be made an employee through HR. In Academics the employee can be made into an instructor in the Instructors top menu section. An instructor is given access to the instructor portal if that is turned on at your institution.
Alumni: Making a Student an Alumni gives the alumni access to the Alumni portal if it is turned on at your institution.

I have notified the programmers. You did it correctly and it should have created a user for you. When I re-clicked the staff portal access and gave the access type, and active I received an error on save. I believe the error is preventing the system from getting to the user creation part. I am working on a workaround for you and will give you an update when I have it. Thank you for taking the time to let us know.
