Bug/Error Found

First, please verify that the degree/major/track is actually assigned to the student.  This can be done in Academics > Students, on the academics, under majors/minors/audits.

If the information is there, please verify that a graduation date is assigned to that entry.  This will appear next to the major.  If there is no graduation date entered, this will cause this problem.  When in edit mode on student > academics, click update next to the degree that is having the issue.  Enter the graduation date and save the student.  Then, re-run the transcript.

I have seen this happen when a user is in two different groups.  Update the user record so that they are only in one group and this should fix the problem

We have seen this happen when tomcat is being run as a service and not as an application.  We are investigating why tomcat as a service runs slow, but until then, run it as an application.

The terms need to be updated.  This can be done in Academics > Administration.  Either the new term isn't in there at all, or it is not marked as the current one.
